We are accepting applications for Qualified PCAs to work with consumers within the metro areas. Individual PCA interested is welcomed to come in and fill out an application. Please call the main office at: 763 - 544 - 1616 for details and directions.
Community Vision Development Programs, LLC is also accepting referrals for services to adults or children in their own homes. We support individuals using Minnesota Health Care Programs including:
- MA Home Care PCA Services (We do provide services for Private Pay)
- In-Home Services (We also accepts In-Home services referrals for Private Pay)
- MR/RC (Mental Retardation/Related Conditions) Waiver
- TBI (Traumatic Brain Injury) Waivers
- CADI (Community Alternatives for Disabled Individuals) Waivers
- EW (Elderly) Waivers
- AC (Alternative Care) Waivers
Our Adult Foster Care Services are designed to meet all types of disability needs. Funding for foster care services is provided for through GRH (Group Residential Housing). Rates are standard GRH rates and Foster Care Consumers have additional staffing services through HCBS (Home and Community-based Service) Waivers which include:(MR/RC, TBI, CADI, & AC Waivers) or Medical Assistance Home Care Personal Care Assistant (PCA) Services.
